What is the process that you go through to ask for time off?

Do you actually ask, "May I have these days off?"

Do you state, "I will need to take the following days off for a vacation,"?

Do you fill out a calendar with the dates that you are planning on taking off penciled in, then e-mail/show it to the boss?
