In your practice, do you use Light’s criteria to dif­ferentiate exudates from tran­sudates in the diagnostic work-up of pleural effusion?

    What happens in our practice is that the doctors /residents send the pleural fluid samples for 'all routine tests' which includes cell count & typing, Gram's Stain, culture sensitivity, if needed AFB staining, Protein & Albumin, ADA if TB is suspected, and of late cholesterol. Such 'routine testing' is a problem everywhere - e.g. all patients get a CBC, complete urinalysis, even though they may not be needed in all patients. I guess pleural fluid testing for Light's criteria falls into that category. It will probably take a long time before attending doctors can give up this habit and spare it only for those cases where it is needed.
