This episode was part of the seasons dark ones. The conflict between the Survivors, the freighter people and finally Ben could have been believed if he had not lied so much already.
At first I didn't think much about Jacks appendix. However after watching all of the seasons, I decided that since Jack was still trying to get off the island and was believing the the freighter was going to rescue them that the Island did it to him, in an effort to stop that foolishness.
He lost the protection from the Island, in the same way the Ben lost it when he developed cancer.
