    quoting 'Elementary Fan':

"I'm confused about the reference to the mole removal also. When Sherlock first mentioned that "Irene's" moles formed a constallation, he said he hadn't noticed that formation previously. Maybe one of the moles had suddenly appeared, and she subsequently became concerned about it. Didn't Sherlock say something about the mole being removed because it was possibly precancerous? Then, did it occur to him that if "Irene" had really been kept captive, she wouldn't have been able to visit a doctor during the period of her captivity? Is this when it occurred to Sherlock that "Irene" was really Moriarity? I'm still confused. LOL."

That actually WAS the explanation given in the episode. While it may at first seem confusing, think about it for a moment... how many kidnappers do YOU know will allow their captors to seek medical attention of any kind? 'Anyone? Anyone? Anyone?...' (crickets chirping) I can't think of any myself, either. It's not likely something that would ever happen. So, if Irene Adler HAD been Moriarty's captive, she either would still have the mole or would've died of cancer. Since she is alive and sans mole, it is safe to assume that she was NOT Moriarty's captive as she had led Sherlock to believe and thus, must herself be Moriarty. Once one puts the pieces together, it does fit into a coherent whole.

One just has to learn to think like Sherlock... :)

Thanks for the question, I hope the answer clears some of the confusion... :)

Can anyone explain why Adler/Moriarity has removed her moles when she reappears to Sherlock? Is it because she loves to mess with his mind? Maybe she did it so that he would be hopelessly confused about whether Irene was dead or alive?
