Yes, too many books out there from Young Adult to Adult Fiction that claim title of "epic fantasy" without adhering to any ground rules.
No, the category's understood enough that a reader knows "epic fantasy" when he or she sees it.
Perhaps, because it would help keep lines clear in a rapidly saturated marketplace where all-comers claim "epic fantasy," but what's in the book isn't defined at all!
Don't know, because haven't given it much thought, and Tolkien and Lewis's worlds should be definition enough.
