Yes, I love getting recommendations from bloggers.
Yes, but only if it's relevant and non-intrusive to the blog.
No, I hate when I go to a blog and it's littered with ads.
No, but if you do it, I would hope it would be tasteful and subtle.
I LOVE give-a-ways. Do those!
Make a separate sponsor, review and give-a-way page that I can click if I want to see it.
