I think it was not intentional but its hard to know either way for sure. The mistake wasn't in the favor of the customer so it seems fishy.

This happens every where and it's sad restaurant s do this . I don't tip unless good service and good food not cold

Does not say much for his or her customers or his help does it. If people get good service,good tips come to them.once in awhile you get people who do not care and will not tip at all.think about it.
