I have when I had my son he was a huge baby. 9 pound 7oz I didn't want any pain meds. My husband really helped I ended up being fully naked in the delivery room I screamed and screamed and screamed i was small figured at the time small hips and was just small it was painful gave about 20 good hard pushes my OB had to fight him she tugged and tugged and tugged finally he out he came after 1 hour of pushing. They said they herd me down the hall. I have sense had 3 more children 2 with pain meds and the last one just 2 months ago this time with my new husband at 48 I never thought I'd be a mom again and I didn't use pain meds and it was painful but no where near bad as my son in 1989.
