What is the mechanism of VT in HCM ?
Bundle branch re-entry
Focal LVOT tachycardia usually at the site of maxium IVS thickness .
It is actually a type of Ischemic VT due to micro vascular disease .
The focus is often in RVOT , Mid ventricle or even LV apex
Exact mechanism is not known .Can arise from any fibrotic , disarrayed myofibril which are clustered at random !
