My response to a grade 4 view of the larynx during RSI of a desaturating trauma patient would be;

Allow muscle relaxant to wear off and transport with basic airway care

Transport with a supra-glottic airway in situ

Remove manual inline stabilisation and attempt direct laryngoscopy again

perform a primary surgical airway
