I tell my doctor all substances I take and how often I take them.

I tell my doctor most of what I’m using and how often.

I tell my doctor 50% or less of what I'm using.

I lie; I say I don’t drink, smoke and/or use drugs, even though that's not true.

I don’t take any substances (and I tell my doctor the truth).
