The aim of this survey is to assess the spiritual health of 5pm - to enable more focussed teaching and training and to
encourage all members of 5pm Church to reflect on their commitment to their Lord and Saviour and their
ministry. The results of the survey will be anonymous, we will not ask for your name or any of personal details.
